Part of metaSOFT HMS

Specialty Clinical Forms is one of 20+ integrated modules on the metaSOFT HMS platform — one database, one patient record, one source of truth.

Specialty Clinical Forms is the documentation layer that sits on top of the EMR. Every completed specialty form is stored in the patient's longitudinal record and is accessible from any other department or facility in the network. A cardiology GRACE score recorded during an emergency visit is visible when the patient attends the outpatient cardiology clinic the following week.

Structured forms are also the foundation of clinical analytics. Because data is captured in defined fields rather than unstructured text, hospital quality teams can query the entire patient population for NIHSS scores above a threshold, Oxford Hip Score trends over six months, or PHQ-9 screening rates by outpatient clinic — without any data cleaning or manual chart review.
